New Client Forms
Completing the forms Venus has asked you to fill out before your first appointment allows more of your scheduled session to be devoted to discussing your concerns, answering your questions, and beginning the therapy process.
You may save the completed forms and email them to venus@lifeatbestcounseling.com before your appointment, or bring printed copies with you to your first session.
If more than one person will be participating in counseling, please complete the requested forms separately for each participant, as instructed by Venus.
Privacy reminder:
These forms contain private personal and health information. Please do not complete or save them on a public or shared computer. Save them only on a personal, password-protected device. If you choose to email the completed forms, make sure they are sent only to venus@lifeatbestcounseling.com and use encrypted email when available, as standard email may not be secure. You may instead bring printed copies to your first session.

What to Expect During Your First Session
At the beginning of your first session, Venus will review confidentiality and relevant practice policies, answer your questions, begin learning about your concerns, and work with you to establish collaborative therapy goals.
