If you’re a new client, please complete the following forms, print and bring them to your first therapy session or fill them online, save and email them back. Please make sure that the four forms below are filled out for each member separately.
If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information:

To best utilize the scheduled therapy time, please fill out the forms in advance and either save them and email them to [email protected] prior to your first appointment or bring the printout of the filled forms with you to the first session.
